Please check the price you should pay in the delegate fee table. I wish to attend Access11 and enclose my delegate details together with payment/official order. Notification of cancellation must be received in writing by 25th February 2011 when a full refund less an administration fee of £25 will be made. The CTA regrets that it cannot make refunds after this date. Any changes to delegate details will incur a £25 administration fee. Information collected on this form will be stored in hard copy and electronic form. The CTA is registered with the Information Commissioners Office and all data collected will be stored and used in compliance with the Data Protection Act. A CTA photographer will attend Access11, please inform a CTA member of staff before or at Access11 if you do not wish any images which may include you to be used.
